Please supply the following

1. Which spot or spots you want to order - Fast, Slow, Fast Pairs etc

2. Dancer Name or name's (2 Names For Pairs)

3. Dancer Number (comp number not adfp number) a brief description is also helpful.

4. Age/Category - i.e. U12 Slow Champ

PLEASE ALSO STATE WHICH EVENT YOUR VIDEO SPOT IS FOR  AS WE ARE AT 2 EVENTS THIS WEEKEND

Any orders that I receive without the correct details will be delayed and I wont have time to chase thing up until I have processed all the ones with the correct details so please dont forget.
